Diabetes Self-Management Program (DSMP)
Self-management programs for people with chronic conditions such as high blood pressure, asthma, arthritis, chronic obstructive pulmonary disease (COPD), diabetes, and many other conditions are a very important part of keeping Ohio healthy. The overall goal of the programs is to enable participants to build self confidence to take part in maintaining and managing their chronic conditions.
The Diabetes Self-Management Program was created at Stanford University and is a proven scientific programs that produce favorable outcomes.
Proven outcomes resulting from attending the classes include; increased healthy behavior, less pain and worry about health conditions, better relationships with healthcare providers, and decreased visits to the emergency room and hospital.
The Diabetes Self-Management class specifically focuses on type 2 diabetes, monitoring blood sugars, and diet, in addition to relaxation techniques, healthy eating, managing sleep and fatigue, managing medications, exercise, and communicating with healthcare providers.
The program is conducted 2.5 hours per week over a 6-week period. Many times the person teaching the class is a volunteer who has already taken the class and benefited from the support and education they have received. Workshops are generally held in community settings such as senior centers, libraries, churches, and hospitals.
The programs can be, but are not required to be, conducted by health professionals; however, each class is conducted by two workshop leaders. The Area Agency on Aging District 7 invites community members to participate in these highly effective classes and is very interested in recruiting any volunteers who may be interested in teaching a workshop series.
