To Become a PASSPORT Provider



Thank You for Your Interest in Becoming a PASSPORT Provider

To be considered as a provider for the PASSPORT Medicaid Waiver Program, you are required to be a formally-organized business or service agency that is actively registered with the Ohio Secretary of State.  If your agency meets this requirement, please continue with the Application Process. In accordance with federal requirements described in 42 CFR 445.460 and per Ohio Administrative Code 5160-1-17.8, the Ohio Department of Aging collects a Medicaid application fee. The fee is currently $688 per application and is non-refundable. The fee applies to agency applicants. The fee will not be required if the agency applicant has paid the fee to either Medicare or another state’s Medicaid provider enrollment program (such as the Ohio Department of Medicaid, the Ohio Department of Developmental Disabilities, or outside of Ohio) within the past two years. However, the Department of Aging requires the agency applicant submit proof of payment with their application.

Application Process

  • Apply online with Ohio Department of Aging (ODA), Provider Certification
    • Long-Term Care Agency Providers.  This type of provider is a legally-organized entity which employs staff to perform the type of services the agency is seeking to be certified. 
    • Long-Term Care Non-Agency Providers.  This type of provider is a legally-organized entity which is owned and controlled by one person and does not employee staff.
  •  ODA will contact The Area Agency on Aging District 7, Inc (AAA7) when a completed application packet has been submitted.
  • AAA7 will contact the agency to conduct a pre-certification review to determine if the agency meets the ODA provider certification: requirements for every provider and Services Specifications (Specs) that were applied for. 
  •  After this review/visit, your agency will receive a Pre-Certification Summary letter.
  • The recommendation to become a certified PASSPORT provider will be sent to ODA. ODA will make the final determination. 
  • If ODA approves the certification, AAA7 will be notified and an agreement will be mailed to the agency for signature.  Once the AAA7 receives the signed agreement from the agency, the agency will be able to receive referrals from AAA7. 
  • If ODA denies certification, the agency will receive notification from ODA and will be ineligible to reapply for certification for one year


If at any time during this process there are changes to your agency or, if you wish to change/add/drop services or counties, you must contact the AAA7’s Provider Relations Department as soon as possible. Any discrepancies in the application material may result in the application being rejected by ODA.
