Care Coordination Program Provider
Area Agency on Aging District 7, Inc. (AAA7) utilizes a competitive provider model for the State of Ohio Community Services funds available for their non-Medicaid Waiver case-managed Care Coordination Program. The AAA7 Care Coordination Program funds Personal Care and Home-Delivered Meals.
The application process to become a Provider for this program occurs every two years on the same schedule as the Request for Proposal process for Older Americans Act Title III programs. Agencies are awarded a two-year contract to provide services.
