Long Term Care Consultations

Purpose:

Everyone should have the option to choose the Long-Term Care that best meets their needs. We provide information and assistance to enable individuals and their caregivers to make informed choices about their Long-Term Care.

Services Provided:

Consultations are provided free of charge with our professional staff in the setting of your choice. Consultations can be provided at your home, one of our offices, at the hospital, at a nursing facility, or other settings as needed. Our consultants will gather information to assist you to identify your Long-Term Care needs and will work with you to locate resources to help meet those needs.

Who is Eligible?

Any person, of any age, who has questions about Long-Term Care services or who needs assistance to locate Long-Term Care services in their community.
